Live Scan Request FormThe signed copy provided by the Live Scan facility. See our Live Scan Requirement page for details.

Government-Issued ID You may use any ONE of the following forms of identification:

Driver’s license (CA or another U.S. state) State-issued ID card Passport (U.S. or foreign)

Important distinction: This ID is only for your CNA training enrollment. The CNA state competency exam has its own separate ID requirement — Credentia currently requires two forms of ID, with at least one being a photo ID. See Credentia’s ID requirements for full details before exam day.

TB Negative ReportDated within the last 12 months. See our TB Screening page for accepted test types.

Physical Exam ReportDated within the last 3 months. See our Physical Exam page for full requirements.
