IMPORTANT: Physical Exam Requirement Before Program Start: Please read the following instructions carefully.
✅ Step 1: Print the Physical Exam Form – Download and print the required PHYSICAL EXAM FORM.
✅ Step 2: Complete the Student Information Section – Fill out the top portion labeled Student Information. The remaining sections must be completed by the licensed physician conducting your physical exam.
✅ Step 3: Schedule Your Physical Exam – You may contact your Primary Care Provider (PCP) and request an annual physical exam. If you have not had one recently, this is typically covered by insurance. Ask your provider to complete the Physical Exam Form during your visit.
✅ Alternative Options if PCP is not a viable option:
- WellnessMart – Charges $85 for a physical exam. Find a location here: WellnessMart Locations
- Local County Health Center – You may contact your local county health center to ask if they offer physical exams. Note: Costs at county health centers are generally higher than WellnessMart.
- Urgent Care Centers – You may also contact nearby urgent care centers to inquire about physical exams. Note: Costs at urgent care centers are typically higher than WellnessMart.
🚫 Important Notes:
- You cannot use a previously completed physical exam form. The State requires a current physical exam dated within the approved timeframe.
- Only forms completed by a licensed medical professional will be accepted.
✅ Step 4: Upload Your Completed Form – Once your physician has completed the form, scan and upload it to our document portal along with all other required documentation listed in Step 4 of the Onboarding Instructions.
