Bay Area CNA School CDPH Application Guide
Do not complete this step before training begins. CDPH requires your training end date on the application, and if you miss a class and need make-up hours, your end date will change. To avoid delays or corrections, submit your application near the end of training once your final confirmed end date is set.
To submit your Bay Area CNA School CDPH application, visit the CDPH Online Portal:
- Select “Certified Nurse Assistant OASYS: Online Submission System.”
- Click on “Online Submission System.”
- On the login page, click “Sign up now” if you don’t have an account.

You’ll be required to upload your ID and Live Scan Request Form, and enter the Provider Number for your training program:
