The Bay Area CNA School Application Form (283B), Student Agreement, and Liability Waiver must be submitted — all available on our Student Portal. These documents are required and must be completed at least 2 days before your class start date, using the same email address you used when enrolling in your CNA class.
You cannot begin CNA training until all of these forms are completed.
