Bay Area CNA School Live Scan Requirement
The Bay Area CNA School Live Scan process is simple: California requires every student to complete a Live Scan (criminal background check) before their program starts. Here’s exactly how to do it — please read carefully.

- Page 1 — Fill out this page with your personal information.
- Page 2 — A sample filled-out form to guide you. Use it as a reference when completing Page 1.
- OCA Number — Enter your Social Security Number in the OCA field, as shown in the sample.
Visit a Live Scan Facility
Take your completed form to any Live Scan facility.
- Most UPS Stores offer Live Scan services.
- Find additional state-approved locations at California Live Scan Locations.
After completing your Live Scan, the facility will give you a signed copy of the “Request for Live Scan Service” form. We need that one page only — just the signed Live Scan request form, nothing else. Upload a scanned copy to our Student Portal.
- Keep this document safe — you will need it again when submitting your CDPH application, as outlined in Step 5 of the Onboarding Instructions.
Important Notes
- 01Processing time: Live Scan results may take a few days to reach the state, but as long as you’ve submitted your Live Scan before training begins, you may start the class.
- 02What to upload: we only need the Live Scan Request Form completed by the facility — we do not need your Live Scan results, since these go directly to the state.
- 03No reuse allowed: you cannot use a previously completed Live Scan. The State of California requires a new Live Scan specifically for CNA training.
Bay Area CNA School Live Scan: Frequently Asked Questions
Live Scan results are usually returned to the state within a few business days, though it can occasionally take longer during high-volume periods. You don’t need to wait for results before starting class — as long as you’ve submitted your Bay Area CNA School Live Scan before training begins, you’re clear to attend your first day.
No. Even if you completed a Live Scan for a previous job, license, or program, the State of California requires a brand-new Live Scan specifically for your CNA training. Reusing an old submission isn’t accepted, regardless of how recently it was completed.
You can complete it at most UPS Store locations, or at any other state-approved Live Scan facility listed on the California Attorney General’s Live Scan Locations page. Bring your printed and completed Live Scan form with you.
The CDPH reviews all Live Scan results directly and makes the final call on certification eligibility — Bay Area CNA School never sees or reviews your results ourselves. Enrollment and completion of training don’t guarantee CDPH approval, and if certification is denied for any reason, tuition and fees remain non-refundable since the training itself was fully provided.
