You must submit California Live Scan (criminal background check) before start of the program. Here are the steps to do Live Scan. PLEASE READ THESE CAREFULLY.
✅ Step 1: Print the Live Scan Form
Download and print both pages of the Live Scan Form.
- Page 1 – Fill out this page with your personal information.
- Page 2 – This is a sample filled-out form to guide you. Use it as a reference when completing Page 1.
- OCA Number – Enter your Social Security Number in the OCA field, as shown in the sample.
✅ Step 2: Visit a Live Scan Facility
Take your completed form to any Live Scan facility.
- Most UPS Stores offer Live Scan services.
- You can also find additional state-approved locations here: California Live Scan Locations
✅ Step 3: Obtain and Upload the Signed Form
After completing your Live Scan, the facility will give you a signed copy of the “Request for Live Scan Service” form.
- Keep this document safe. As outlined in Onboarding Instructions, you’ll need to upload a scanned copy of this document to:

- The CDPH Online Portal
⚠️ Important Notes
- Processing Time: Live Scan results may take a few days to reach the state, but as long as you’ve submitted your Live Scan before training begins, you may start the class.
- What to Upload: We only require the Live Scan Request Form completed by the facility.
- We do not need your Live Scan results — these are sent directly to the state.
- No Reuse Allowed: You cannot use a previously completed Live Scan.
- The State of California requires a new Live Scan specifically for CNA training.
- Background Check & Certification Disclaimer
- The California Department of Public Health (CDPH) is solely responsible for reviewing Live Scan background checks and determining eligibility for CNA certification. The school does not receive, review, or have access to any student’s Live Scan results.
- Enrollment in the CNA program and completion of training does not guarantee approval by CDPH or eligibility for certification.
- If CDPH denies certification due to criminal history or any other reason, tuition and fees are non‑refundable, as the school has fulfilled its obligation to provide the training program.
