Deadline to Submit the Physical Exam Report

Print the Physical Exam Form
Download and print the required Physical Exam Form.
Complete the Student Information Section
Fill out the top portion labeled Student Information. The remaining sections must be completed by the licensed physician conducting your physical exam.
Schedule Your Physical Exam
Contact your Primary Care Provider (PCP) and request an annual physical exam. If you haven’t had one recently, this is typically covered by insurance. Ask your provider to complete the Physical Exam Form during your visit.
A Doctor, Nurse Practitioner (NP), Physician Assistant (PA), or School/College Health Center Provider are all authorized to perform and sign physical exams.
Alternative options if a PCP isn’t a viable option:
- WellnessMart — charges $85 for a physical exam. Find a location at WellnessMart Locations.
- Local county health center — contact your local county health center to ask if they offer physical exams. Costs are generally higher than WellnessMart.
- Urgent care centers — contact nearby urgent care centers to inquire about physical exams. Costs are typically higher than WellnessMart.

- Only forms completed by a licensed medical professional — Doctor, Nurse Practitioner (NP), Physician Assistant (PA), or School/College Health Center Provider — will be accepted.

Bay Area CNA School Physical Exam: Frequently Asked Questions
We accept a physical exam report dated within the last 3 months, so a recent exam from your annual checkup or another program may still qualify. If your exam is older than 3 months, you’ll need a new one completed before submitting it.
Only a licensed Doctor, Nurse Practitioner (NP), Physician Assistant (PA), or School/College Health Center Provider can complete and sign the form. Exams signed by anyone outside these categories won’t be accepted.
WellnessMart offers physical exams for $85 with locations across the Bay Area. Local county health centers and urgent care centers are also an option, though they typically cost more than WellnessMart — call ahead to confirm pricing and availability.
The deadline depends on your program: 2 weeks after your first day for Weekend students, or 5 business days after your first day for Weekday AM or PM students. Missing the deadline for your Bay Area CNA School Physical Exam can affect your ability to continue in the program, so it’s best to schedule your appointment as early as possible, ideally before your first day of class.
