OnBoarding Instructions

Please find below important onboarding information. Make sure you have completed the registration for the CNA class before you start the onboarding process.

STEP 1: Read following very important policies –

Transfer, Cancellation and Refund Policy – Please refer to our Refund Policy for more details.

Makeup class policy – Please refer to our Make-up Policy for more details.

STEP 2: Fill out the CNA application form on our website. *** THIS IS VERY IMPORTANT.  IT IS DUE 3 DAYS BEFORE THE START DATE OF TRAINING  ***

STEP 3: Live Scan – You must submit California Live Scan (criminal background check) before start of the program.  Visit Live Scan page for more details.

STEP 4: Documentation

All the following documents must be uploaded here. We are only accepting these documents digitally. Do not send them in email, or bring a paper copy. Only way to send these documents is through the upload link above.

  • Signed copy of Live Scan Request Form from Step 3.
  • COVID vaccination card(s). (due before 1st day of class). – Optional. If you not vaccinated, you can ignore this.
  • Copy of your driver’s license or state ID. (due before 1st day of class)
  • Copy of your TB negative report. (due one week after your class start date). We can accept a previous TB negative report if it is within last 12 month.

Please make sure to save scanned copies of your ID and Live Scan Request Form, as you will also need them in Step 5, when submitting CNA application on CPDH online portal.

STEP 5: Submit CNA Application on CDPH online portal. This should be done in the 1st week AFTER you have started the training.

You will need to go to the first option that says “Certified Nurse Assistant OASYS: CNA Initial Application” (see the picture below). Click on the link that says “Online Submission System”. Once you click on it, you will be taken to a login page. When submitting it for 1st time, you will need to create an account. (Click on “Sign up now” link next to the text – “Don’t have an account?”).

When filling out online application on CPDH portal, you will be asked to upload your ID as well as live scan request form. You will also be asked to enter “Provider Number” for the training facility. Please use the table below to find the correct Provider Number for your program.

Other important information:

1. All classes are in-person. Morning classes start at 7 AM, while PM classes start between 2:30 and 4 PM. Please check the details of your program for exact dates and hours.

2. If you signed up for the optional BLS / CPR training, please go to this link and complete registration for the BLS class. When you start the checkout for BLS class it will show you a fee of $100, but you can enter promo code BayAreaCNA-BLS-Addon in the last step of the checkout process to waive off the fee for BLS class. (Please note this is only for students who added BLS class as a add-on when they enrolled for CNA class, and have already paid for it)

3. Our school address is “7567 Amador Valley Blvd, STE 108, Dublin, CA”. (it is right across Habit Burger Grill). We have 2 class rooms.

4. You will be provided a curriculum book and a scrub set on your first day of class. There is no dress code for the theory classes. However, you must wear the school provided scrubs during your clinical training. 

5. You can pay the remaining balance by credit card, debit card, check or cash. Check or cash payments can only be made in office. Checks should be made payable to “Bay Area CNA School”.
