You will need to print Live Scan form and fill it out before you visit the live scan facility. Please leave the employer section blank. 2nd page of this document has a sample filled out form for your help. It will show you how to correctly complete the form.
Please note that we only need the Live Scan Request form completed by the live scan facility. We do not need your Live Scan results, these are directly sent by the live scan facility to the state.
